The Jordan Smith Community Care Foundation exists to bridge these gaps by supporting pharmacist-led innovation, expanding access to care, and strengthening community-centered health solutions.
We support pharmacists, students, and healthcare leaders with the confidence, resources, and opportunities needed to lead meaningful change for themselves and for the communities they serve.
Our work is built on a multiplier approach to community health. Meet the Founder
Jordan Smith, PharmD is a pharmacist, leader, and advocate committed to shaping the future of healthcare by expanding what comes after the prescription.
Through her experience in community pharmacy and community-based care, Jordan witnessed both the extraordinary impact pharmacists can have and the structural barriers that often limit innovation, leadership, and access to care. She saw how these gaps affect not only healthcare professionals, but also the communities they serve, particularly those facing inequities in access, health literacy, and trusted, community-centered care.
Jordan’s work is grounded in the belief that pharmacists are more than medication experts. They are essential leaders in community health, education, and system-level change. She is deeply committed to supporting underserved communities and empowering healthcare professionals to design solutions that improve access, strengthen trust, and create meaningful, lasting impact.
The Jordan Smith Community Care Foundation was created from this lived experience. Jordan founded the organization to support pharmacists, students, technicians, and emerging healthcare leaders with the resources, education, and confidence needed to build innovative care models, develop sustainable services, and expand the reach of community-centered healthcare.
As Founder and Executive Director, Jordan provides strategic leadership, program oversight, and long-term vision for the Foundation, ensuring initiatives remain mission-driven, accountable, and responsive to the evolving needs of both the profession and the communities it serves. Her work brings together leadership development, entrepreneurship, and community advocacy, rooted in the belief that when healthcare professionals are supported, communities thrive.
This work is personal, and it continues to be shaped by the communities and professionals it serves every day.
